Insurances accepted:

Healing Wounds, LLC is in network with the following insurance companies:

Aetna
Tricare
United
AHCCCS (Mercy Care, REBH, United Community Care, DDD) - Psychiatric Services are not covered
VA Choise/TriWest/CCN
Humana
Cigna
MHN
Medicare

Please always first contact your insurance company to insure in network benefits - each insurance company has multiple different plans and each have different rules.

For all other out of network, a statement can be provided that you may submit to your insurance company, but cannot guarantee reimbursement of all or part of the fee. Please contact your insurance company for details on benefits for an “out of network” provider.

Fees:
Cancellation Policy – There is a $120 Cancellation Fee for any cancellation made within 24 hours of a scheduled appointment.

Assessment/Evaluation: Evaluations are $300 per hour.
Fees apply to time spent interviewing, administering/scoring tests, reviewing documents, telephone conversations, conferences, and correspondence. One-hour feedback (if relevant) for completed evaluations is included in the cost of the evaluation.

Therapy
Individual Therapy

$150/hr. for master’s level clinician
$300/hr for senior psychologist

Group Therapy

$50/hr

Court Fee: We require a retainer of $2000 at a rate of $550.00 an hour for local court (Phoenix/Scottsdale Area).

Psychiatry

Initial Intake

$250 - requires a $50 deposit to schedule. The deposit is non refundable for no shows and same day cancelations

Medication Management Appointment (follow up appointment)

$90 for 20 min appointment
$150 extended appointment up to 30 min additional time
$200 extended appointments up to 60 min

Medical Records (requested by patient):

Paperwork outside of session (FMLA, SS Disability, Disability or School or Employment related)

$150 (or faction there of if it takes less than 30 min to complete)
